Clinical Classification of Cerebral Palsy

Cerebral palsy CP in children is highly prevalent. Despite this fact, however, nutritional care in CP children has not been given much of the research attention. Nutrition is usually overlooked for many reasons, but mostly because the medical physicians are usually not skilled in nutritional care. This gap needs to be filled through numerous approaches. In this explanatory review, we present a brief overview on CP children and their nutritional care; how is growth and nutritional status assessed in CP children? What are common feeding problems of CP children?

Cerebral palsy is a common neurodevelopmental condition encountered by pediatricians. The condition may present itself in many diferent clinical spectra. The etiological and risk factors are many and an awareness of the interplay of multiple factors in the causation of CP is crucial. In many cases, the cause of Cerebral palsy may not be apparent. Cerebral palsy is invariably associated with many deficits such as mental retardation, speech and language and oromotor problems.

Cerebral palsy CP is a group of permanent movement disorders that appear in early childhood. Cerebral palsy is caused by abnormal development or damage to the parts of the brain that control movement, balance, and posture. CP is partly preventable through immunization of the mother and efforts to prevent head injuries in children such as through improved safety. Cerebral palsy is the most common movement disorder in children. Cerebral palsy is defined as "a group of permanent disorders of the development of movement and posture, causing activity limitation, that are attributed to non-progressive disturbances that occurred in the developing fetal or infant brain.

The classification of cerebral palsy CP remains a challenge; hence the presence of so many classifications and a lack of consensus. Each classification used alone is incomplete. The recent WHO International Classification of Functioning, Disability and Health ICF emphasizes the importance of focusing on the functional consequences of various states of health and has stimulated the development of newer functional scales in CP. Because of the availability of new knowledge about the neurobiology of developmental brain injury, information that epidemiology and modern brain imaging is providing, the availability of more precise measuring instruments of patient performance, and the increase in studies evaluating the efficacy of therapy for the consequences of injury, the need for reconsideration of the definition and classification of cerebral palsy CP has become evident. Pertinent material was reviewed at an international symposium participated in by selected leaders in the preclinical and clinical sciences. Suggestions were made about the content of a revised definition and classification of CP that would meet the needs of clinicians, investigators, and health officials, and provide a common language for improved communication. With leadership and direction from an Executive Committee, panels utilized this information and have generated a revised Definition and Classification of Cerebral Palsy. The Executive Committee presents this revision and welcomes substantive comments about it.
